Warehouse Risk Assessment Template | Excel WHS Risk Register

About this template
This warehouse risk register turns a static safety checklist into a working Excel model. Enter a likelihood and consequence for each hazard and the workbook reads the risk band straight off a 5×5 risk matrix, then lifts it where a plain matrix understates the danger — five override rules cover fatality-credible hazards, sole reliance on administrative or PPE controls, repeat control failures, exposure of casual and agency workers, and unrepaired racking damage.
A built-in hazard library of 32 warehouse hazards — mobile plant and pedestrians, racking, working at height, manual handling, loading dock, fire and lithium-ion, automation, and psychosocial risk — feeds the register automatically. Controls are credited by the hierarchy of control, so an evidenced engineering control reduces residual risk while an unverified one earns nothing. Everything above tolerance flows to a findings and actions register with owners, due dates and automatic open/overdue/closed tracking.
The risk dashboard summarises the whole assessment at a glance: KPI cards, a residual-risk heatmap, and charts for inherent-vs-residual bands, override impact and findings status. It suits safety managers, warehouse and operations managers, and HSE consultants who need a defensible, auditable assessment.
